不同引流方法对跟骨骨折手术切口愈合的影响

摘要:
目的 分析比较皮片引流、引流管低负压引流和封闭式负压引流对跟骨骨折手术切口愈合的影响。方法 前瞻性研究2015年1月—2018年1月陆军军医大学大坪医院关节四肢外科收治的跟骨骨折手术患者126例,男性96例,女性30例; 年龄18~60岁,平均 36.4岁。 Sanders分型:Ⅱ型32例,Ⅲ型74例,Ⅳ型20例。采用随机数字表法分为皮片引流组、低负压引流管引流组和封闭式负压引流组,各42例。均行经跟骨外侧L形切口,切开复位钛板内固定术。比较三组患者的引流量、切口愈合情况等。结果 封闭式负压引流组引流物放置时间(112.37±29.14)h长于皮片引流组(41.21±5.21)h和低负压引流管引流组(67.54±3.37)h,差异有统计学意义(P<0.05)。封闭式负压引流组引流量(482.71±65.79)mL多于皮片引流组(84.72±38.46)mL和低负压引流管引流组(183.03±75.28)mL,差异有统计学意义(P<0.05)。皮片引流组切口愈合时间(21.59±3.32)d长于低负压引流管引流组(13.56±7.69)d和封闭式负压引流组(12.59±2.46)d,差异有统计学意义(P<0.05)。皮片引流组切口边缘皮肤坏死9例,坏死率21.4%,感染6例,感染率14.3%。低负压引流管引流组切口边缘皮肤坏死4例,坏死率9.5%,感染2例,感染率4.8%。封闭式负压引流组切口边缘皮肤坏死4例,坏死率9.5%,无感染发生。结论 与皮片引流相比,引流管低负压引流和封闭式负压引流的效率更高,切口并发症发生率低,切口愈合快。而其中封闭式负压引流切口感染率最低,具有较大的临床应用优势。
