摘要:

目的 探讨髓内钉(IMN)治疗胫骨干骨折的隐性失血量(HBL)及相关影响因素。方法 回顾性分析2017年1月—2019年1月山西医科大学附属第二医院骨科采用闭合复位IMN内固定治疗胫骨干骨折患者81例,男性65例,女性16例; 年龄19～84岁,平均45.4岁。统计患者术中失血量并计算其术后第1、3天HBL,分析性别、年龄、BMI、骨折分型、糖尿病、高血压、伤后至手术时间、手术持续时间并筛选出危险因素。结果 术后第1天HBL平均(160.43±131.42)mL,占总失血量的67.47%; 术后第3天HBL平均(338.23±85.24)mL,占总失血量的81.39%。性别、BMI、骨折类型、糖尿病、高血压是导致HBL的危险因素(P<0.05)。结论 IMN治疗胫骨干骨折术后失血以HBL为主,手术前应评估患者的一般情况,尤其是合并糖尿病、高血压的肥胖患者,术后更容易出现较严重的HBL问题。
