活动性脑脊液漏的3DCT诊断和手术修补治疗

- 目的探讨颅底骨折并发活动性脑脊液漏的诊断方法和手术要点。方法收集2005年 2008年共11例颅底骨折并发活动性脑脊液漏患者的3DCT资料和临床资料，对比3DCT表现与术中情况，探讨诊断的准确性和手术治疗的有效性。11例均行开颅硬膜下手术，以自体肌肉、筋膜、骨膜、人工硬膜等材料结合生物胶修补漏口，关闭硬脑膜腔。结果11例术前均行3DCT颅底检查，影像学检查发现的漏口与术中探查漏口位置相符。11例患者术后随诊3 6个月，无脑膜炎及脑脊液漏复发。结论3DCT对颅底骨折并发活动性脑脊液漏的患者能够在术前提供必要的手术信息，开颅行漏口修补手术是治疗颅底骨折并发活动性脑脊液漏确切有效的办法。
