摘要:
踝关节骨折是创伤骨科医师最常见的损伤之一。制定踝关节骨折手术固定的方案需要了解关节的骨性和韧带解剖结构以及正常的踝关节生物力学。这些损伤的手术治疗旨在解剖复位、坚强固定以及允许尽快开始关节活动。近几年较受关注的包括外踝骨折、孤立的内踝骨折、三踝骨折中后踝骨折的固定以及踝关节镜的应用。就外踝骨折的治疗而言,锁定钢板能够提供更好的解剖复位; 对于年轻患者效果最好,老年患者更适合髓内固定。内踝骨折由于其较高的不愈合风险以及长期持续移位会导致距骨倾斜,损害踝关节的生物力学,手术治疗可能是一个更好的选择; 全螺纹空心无头加压螺钉固定可能是未来趋势。后踝骨折治疗方案的制定除了要根据骨折块的大小,更重要的是要根据踝关节内关节面是否存在台阶。由于后踝的重要性及其与下胫腓联合的密切联系,后踝骨折的治疗可采取更积极的手段。踝关节镜是一种非常有价值的手术工具,对于关节内骨折的精确诊断和修复均有帮助。
